Data Subject Application Form

Under Article 11 of the Turkish Personal Data Protection Law No. 6698 (“KVKK”), you may submit your requests regarding your personal data to SoleLuna Tech, acting as data controller, through this form.

1. Application Methods

• By e-mail: from the e-mail address registered in our system to contact@solelunatech.com,
• In writing: with a signed petition delivered in person or by post to our company address below,
• Via registered electronic mail (KEP) / secure electronic signature.

2. Information Required in the Application

• Full name and, if the application is in writing, your signature,
• National ID number for Turkish citizens; nationality and passport/ID number for foreign nationals,
• Residential or business address for notification,
• E-mail address, telephone and fax number (if any) for notification,
• The subject of your request and related explanations.

3. Your Rights (KVKK Art. 11)

As a data subject, you have the right to learn whether your personal data is processed; to request information if it has been processed; to learn the purpose of processing and whether it is used in accordance with that purpose; to know the third parties to whom data is transferred domestically or abroad; to request correction of incomplete or inaccurate data; to request erasure or destruction of data in accordance with the law; to request that these actions be notified to third parties; to object to a result against you arising from analysis solely by automated systems; and to claim compensation for damages arising from unlawful processing.

4. Response to Your Application

Your application will be concluded free of charge within at most 30 (thirty) days depending on the nature of the request. If the process requires an additional cost, a fee may be charged according to the tariff set by the Personal Data Protection Board. Your identity information must be accurate and up to date so that your request can be assessed.
